1.) Student brings five (5) copies of thesis/dissertation to the Library in individual envelopes with name on outside along with GS form 15 (this form may be supplied by the library). GS Form 15 must be filled out and signed and then returned by the student to his/her graduate advisor. 2.) Student is informed of the cost of binding the these/dissertation and given a Louisiana Tech Departmental Deposit Ticket and sent to the Comptroller payment window to deposit payment according to the following scale:
3.) Student fills out an address label with the address that he/she would like to have the personal copy mailed. The label is filed and kept until the bound copies are returned from the bindery and the student's copy can be mailed. 4.) Library employee receiving thesis/dissertation fills out "Approved Thesis/Dissertation Form". This form remains in the files for future reference if needed. 5.) Student submitting a dissertation to be copyrighted must pay an extra charge (determined by UMI). The student is asked to make out a check payable to PQIL. This check is given to the library employee receiving the dissertation and placed with ProQuest Form PQUMI0809-DISS to be forwarded to cataloging. 6.) Dissertations must be accompanied by ProQuest Form PQUMI0809-DISS whether the dissertation is copyrighted or not. The student must fill out the form and sign on the back (additional signature is required if dissertation is to be copyrighted plus the check). This form also requires a copy of the dissertation title page and abstract. |
